  • Special Salary Adjustment or Bonus Payment Form

    • Faculty may apply for a special salary adjustment or bonus payment by submitting a completed Special Salary Adjustment or Bonus Payment form to the appropriate Dean or Director. See Reference 18, Memorandum of Understanding on the Procedures for Special Salary Adjustments and Bonus Payments.
  • UH-Manoa Legal Services Request Form

    • Form used to request legal services. (Article III. D. Legal Representation)
  • UHPA Expense Voucher Reimbursement Form

    • Voucher for reimbursement for UHPA travel.
  • UHPA Membership Application

    • Membership in the UH Professional Assembly (UHPA) is by choice. Bargaining Unit 07 members only become an Active UHPA member when an UHPA membership application is completed and signed.
