UHPA Active Members get a $20,000 life insurance policy – FREE

Effective January 1, 2018, the no-cost $20,000 Term Life Insurance benefit coverage for all active UHPA members that started back on August 1, 2013 has transitioned from Royal State Insurance Company to Pacific Guardian Life (PGL), ensuring no lapse or break in coverage for our existing policyholders.

PGL is the largest domestic life and disability insurer in Hawai‘i, with corporate offices located here in Honolulu, and is a wholly owned subsidiary of Meiji Yasuda Life, one of the worlds’ largest life insurance companies.

For the basic $20,000 no-cost policy, members should complete the “Participant & Beneficiary Information” sections of the APPLICATION FORM.  Be sure to sign, date and return this form to Pacific Guardian Life, 1440 Kapiolani Blvd, Suite 1700, Honolulu, HI  96814.

As a member covered under the PGL Life Insurance Group Policy, you have access to the Certificate of Coverage and the following programs: (1) AXA Travel Assistance Program which offers a broad range of worldwide medical and travel assistance services; and (2) ComPsych Guidance Resources which offers around-the-clock access to confidential bereavement counseling, financial and legal guidance, and other services.
