NEA Retired (Pre Retired) Life Membership

Complete the attached form and submit it with your check payable to UHPA for $250. UHPA will process your membership with the NEA. Please note that as a NEA Pre-Retired member, you are not deemed eligible to participate in NEA Member Benefit programs until after your retirement at which time your NEA Retired Lifetime membership begins.

Special Salary Adjustment or Bonus Payment Form

Faculty may apply for a special salary adjustment or bonus payment by submitting a completed a Special Salary Adjustment or Bonus Payment form to the appropriate Dean or Director. See Reference 18, Memorandum of Understanding on the Procedures for Special Salary Adjustments and Bonus Payments.

UHPA Reimbursement Policy & Expense Voucher

Included in this folder is the UHPA Reimbursement Policy & expense voucher. Please complete and sign the expense voucher and mail it to UHPA. Don't forget to include your receipts if applicable.

UHPA Membership Application

Membership in the UH Professional Assembly (UHPA) is by choice. Bargaining Unit 07 members only become an Active UHPA member when an UHPA membership application is completed and signed.
